HEALTH INFORMATION FROM THE NURSE

Dear Parents of 7th Grade Students:

Students entering 7th grade are required to show proof of a Tdap vaccination, after they turn 12 years old, prior to entering 7th grade. At the time your student receives their Tdap, your health care provider may wish to immunize your student for Meningococcal and Human Papillomavirus diseases.

According to Washington state law, students who have not provided verification of the Tdap vaccine or a valid exemption form on file will be excluded.

If your student has already received the Tdap vaccine please send this information to school so we can update our records and avoid exclusion.

If you have signed a personal/religious exemption for other vaccines and you wish to sign an exemption for the Tdap please come to the office to get another exemption form that must be signed by you and your health care provider.

If you have any questions, please contact the school nurse at your school.
